Description:
Job purpose:
To contribute to a high quality, co-ordinated and needs-led Carer Support service for carers, and their families, resident in Sefton, through the provision of a dedicated, responsive and accessible Duty service.
Key Duties and Responsibilities:
· To work in partnership with Reception Staff and the wider Carers Support Team to support carers through unscheduled drop-ins to the Carers Centre, telephone and email enquiries and pre-arranged Duty appointments.
· To respond sensitively to unexpected situations, enquiries and referrals from carers, family members and external agencies.
· To register new carers with the Carers Centre and provide information, advice and guidance, including signposting to additional services.
· To undertake assessments of carers and their needs and, where appropriate, refer carers to the wider Carer Support Team to access additional support.
Skills, Qualifications and Knowledge:
Knowledge and understanding of current legislation which underpins Health and Social Care, including the Care Act 2014Awareness of principles of equal opportunities and a commitment to anti-discriminatory/anti-oppressive practice
Previous experience of managing a caseload
High level report writing skills, with the ability to translate information conveyed verbally into the written form.
Excellent communication skills, both written and verbal
Ability to liaise at all levels
Good self-management skills, with an ability to work under own initiative
Ability and commitment to maintaining confidentiality of information relating to the organisation
Excellent ability to use and understand Information Technology including Microsoft software packages, Outlook, Word and Excel, as well as accessing and maintaining database entries
