Job Purpose: To work with schools, colleges, Connexions, children’s services and other organisations to provide a support service to young carers

Main Responsibilities: To form close links and partnership working relationships with services and organisations working with young people aged 16 – 18 years to develop referral pathways. To develop a Peer Support group of young carers who have transitioned for the benefit of both young carers and young adult. To undertake transitions assessments and support planning for young carers To work jointly with the schools and colleges in promoting and facilitating the identification of 50 young carers per year to plan transition pathways

Working Pattern: 28 hours per week (including some evening & weekend working)
