Job Purpose: Based in all Sefton Areas. Working in the community to help people regain life skills by working alongside them and promoting independence. Driver with access to own vehicle is essential.

Main Responsibilities: Supporting individuals who have been discharged from hospital or may have had an illness, crisis, fall at home or change in circumstances in relation to themselves or their carers. The main aim is to help individuals gain confidence by working alongside them. More detailed Job description available below.

Working Pattern: Day, evening and weekend shifts, therefore must be flexible.

Hours of Work: Part Time

Minimum Salary: £9.39

Maximum Salary: £9.57
