Job Purpose: To enable service users to regain or maintain their ability to live independently in the community by supporting them to achieve their agreed outcome-focussed reablement goals as set out in their support plan.

Main Responsibilities: To improve discharge and transfer from hospital and help to reduce avoidable admissions/readmissions to hospital and residential care. supporting individuals who have been discharged from hospital or may have had an illness, crisis, fall at home or change in circumstances in relation to themselves or their carers.

Working Pattern: Day, evening and weekend shifts, therefore must be flexible. Relief work is also available.

Hours of Work: Part Time

Minimum Salary: £9.39

Maximum Salary: £9.57
