Job Purpose: Working in all areas of Sefton. To improve discharge and transfer from hospital and help to reduce avoidable admissions/readmissions to hospital and residential care. This is an exciting time to join a new team that will enhance the well-established Reablement service. Driver with access to own vehicle is essential.

Main Responsibilities: To enable service users to live independently in the community by supporting them to achieve their agreed goals as set out in their support plan. A Full Job description is available below.

Working Pattern: Day, evening and weekend shifts, therefore the post-holder must be flexible. A driver with access to own vehicle is essential.

Hours of Work: Full Time

Minimum Salary: £9.39

Maximum Salary: £9.57

Comments: An Enhanced DBS Check is required for this post.
